The #1 Manufacturing Mistake many new PCB Designers make is assuming the CAM Files output from their PCB Design software tools are perfect and ready for manufacturing handoff. It's hard not to think that way -- because their PCB Design passed DRC and looks perfectly displayed on the high-resolution monitor.
There has Never been a PCB Design tool (commercial or open-source) that always output perfect CAM files!
If this sounds like an absurd statement, choose your favorite PCB Design software tool and review their Revision History (Change log) and see how many times reported bug fixes, changes and alterations were made their CAM outputs.

Ultimately, performing a DFM Check every time on your outputted CAM files (and Passing), will drastically reduce the possibility of PCB Manufacturing issues and failures.
