I tried adding a pad by clicking on the “Pad” icon. It requests a location for the center but after I place it I do not see a pad. What am I missing? How do I size the pad and how do I assign it to a layer?

Here's a quick tutorial (no sound) which shows how to add a Pad.

Hint: FAB 3000 is a Gerber editor, which means when adding Pads or Traces you must first define the Aperture (i.e Shape/Size) and then select it.  My guess is in your example the Pad was added, however you may have used a very small aperture.
